Important Information For Your Appointment 

  • If you are having allergy testing, you must not take any antihistamines for at least 3 days prior to your appointment. Common anithistamines include: Atarax, clarinex, claritin, zyrtec, allegra, benadryl, astelin, astepro, patanase, all over the counter allergy and cold medications, over the counter sleep aids such as Tylenol PM.

  • Please continue to take all inhaled medications.

  • Due to our testing procedures, if you are a patient with long hair please bring the necessary clips to tie back your hair.

  • Your first appointment may take up to 2 hours

  • We ask that you not mail the patient registration forms. Please bring the completed forms with you to your visit.

  • Please bring your insurance identification cards(s).

  • All patients must present their insurance card(s) and a valid picture ID in order to be seen on the day of the appointment.  We are required to verify patient/guarantor information from a valid picture ID.

  • Any portion that is not covered by your insurance (copay, coinsurance or deductible) is due at the time of service.

  • If you are covered by an insurance plan that requires an authorization in order for you to be seen by a specialist, please contact your primary care physician prior to your appointment for authorization. Your primary care physician may fax your records and/or authorization to: 978-256-1377.

  • Bring a complete list of ALL current medications.
