Appointments 

 

New and follow-up office visits are by appointment only, with emergency visits available when necessary. Cancellations should be made with at least 24-hour notice. No appointments are needed for allergy shots.  Allergy shots administration hours are posted in all offices. Click here for directions to our offices and shot hours.

 

Patient Privacy

 

Our office is compliant with the latest HIPPA guidelines. All patients will receive a copy of our privacy policy – or have a link to download this form.

 

Prescription Policy

  • Prescription refills are generally valid for a maximum of three months.
  • Patients must have been in the office within the past year in order to have a prescription refilled.
  • Patients calling in for a refill beyond the one year period must make an appointment; then they will receive a limited prescription to sustain them until the appointment (one time only).
  • Patients with mail-in refill requests over the one year period will be given the full prescription as long as they schedule an appointment (one time only).
  • Routine prescription refills that cannot wait until regular office hours will be accepted by our answering service only between the hours of 9:00 am to 12:00 noon weekends and holidays. At other times, the doctors are on call for emergency illnesses only.
  • Patients with new complaints such as sinusitis must schedule a doctors visit if the office is open. Every effort will be made to see the patient on the same day. Patients who receive treatment from a phone triage when the office is closed must schedule a followup office visit at their earliest convenience.

Fees, Insurance and Managed Care

 

Where applicable, we will submit insurance forms and bill you for the remainder. Managed care patients must co-pay at the time of their visit and have valid referrals (to avoid being personally responsible for the bill). Payment by patients without insurance is due on the day of services unless billing arrangements have been made prior to the date of service.

 

Fee schedules for visits and services are available at the reception desk. Patient account representatives are available to assist with billing and insurance questions at our Chelmsford office and can be reached at 978-256-9528.
